I ended up reformatting back to the original OS and upgrading to El Capitan, but not Sierra and had the same issue again. The next step I'm going to try is reformatting and not upgrading to Sierra.

Does anybody have other ideas as to what could be causing this? It was working fine until the upgrade to macOS Sierra, so it could be the OS itself, but that just seems unlikely. I suppose macOS Sierra could be the problem, so I'm looking at reformatting and upgrading just short of macOS Sierra. I'm a little lost here because I've ruled out the file as an issue and I've ruled out the machine as the issue.
